The Latino Health Insurance Program (LHIP) has been receiving technical assistance from NCHH and other partner experts to explore sustainable mechanisms to maintain their asthma home-visiting program that utilizes a CHW-based model program to target high-risk clients through physician/clinical referrals. Challenges related to securing sustainable funding streams, referral volume, overall community awareness of the available resources, and internal capacity related to potentially available funding mechanisms have been the primary focus of the support provided.
